Care Consultation provides you with an opportunity to speak with a care consultant. If you are struggling with care-related decisions or just have questions this is your chance to talk with a care consultant via the telephone. Our geriatric care consultants are trained and ready to help provide you with answers to the most common care questions.
To activate geriatric care services, you must first purchase a retainer package of at least 4 hours (which includes an in-home assessment and evaluation and one hour of care management). To do this, change the quantity of care management hours to "4" after adding it to your shopping cart.

We understand that everyone’s situation is different and that care management services may need to be used in an assortment of ways. Terms & Privacy Policy for Senior Care Solutions Online Geriatric Care Management Services
Prior to purchasing any geriatric care management services we recommend that you contact our office at 1-888-788-1056. Geriatric care management services may not be available in your area.
1. ACCEPTANCE OF TERMS
Welcome to Senior Care Solutions Online (SCSO). SCSO provides its service to you, subject to the following Terms. Terms may be changed by SCSO from time to time without notice.
A written contract that is signed by both parties must be in place before geriatric care management service can begin. The terms below are not intended to be, and do not represent, a contract. They are for informational purposes only.
2. DESCRIPTION OF SERVICE
SCSO currently provides users with a collection of on-line resources for senior care and related senior information. You understand and agree that the Service is provided "AS–IS" and that SCSO and staff cannot guarantee the quality or credibility of any of the communities or resources listed with us.
In order to use SCSO, you must obtain access to the World Wide Web, either directly or through devices that access web-based content, and pay any service fees associated with such access. In addition, you must provide all equipment necessary to make such connection to the World Wide Web, including a computer and modem or other access device.
3. YOUR OBLIGATIONS AND EXPLANATION OF FEES
In-home Assessment and Evaluation for a fee of $360.00
This includes an initial visit to a Client located within a thirty-mile radius of the assigned care managers office, assessment and evaluation of the Client's mental, physical and financial needs, development of a written report that identifies available resources and providers, and initial referral to programs and/or providers identified by the care manager.
Services at the rate of $90.00 per hour, with an initial retainer and cost deposit of $650.00.
Premium rate - $135.00 per hour (please see written contract for more details on premium hourly rates).
Travel outside of a 30-mile radius of the assigned care manager will be billed at .50 per mile.
Management services include all time spent by the assigned care manager on behalf of the Client. This includes telephone calls, visits, correspondence, re-applications and continuation of eligibility requirements for programs identified in the written care plan, applications or other contact with the Client, family members, friends, and/or providers, including travel time. The Client is also responsible for costs and out-of-pocket expenses incurred by the assigned care manager on their behalf. Management services provided by assigned care manager are billed in 15-minute intervals, with a 15-minute minimum. Senior Care Solutions Online sends periodic invoices to the Client or responsible party. Invoices that are not paid within thirty days are subject to a late fee of 1 1/2 percent per month, and may result in cessation of all geriatric care management services pending satisfactory financial arrangements.
While Senior Care Solutions Online strives to refer to only providers of high quality services, Senior Care Solutions makes no representation of, and does not warrant or guarantee the credentials, professional qualifications, experience, services, and/or advice of any third party. Should a geriatric care manager be assigned it is the responsibility of the responsible party to interview any assigned geriatric care manager and verify the credentials, professional qualifications, liability insurance and experience. Purchaser and Client agrees to hold harmless Senior Care Solutions Online and/or any of its agents, for any actions including actions of harm to persons or financial loss due to actions of any third party provider and any assigned geriatric care managers. The Client is responsible for investigating and evaluating programs, providers, and is solely responsible for their charges. The Client agrees to indemnify Senior Care Solutions Online and/or any of its agents for any liability or costs arising out of third party claims, and for any costs of collections incurred by Senior Care Solutions and/or its agents, including reasonable attorney's fees. Any person purchasing geriatric care management services through Senior Care Solutions Online is representing that they are the "responsible party" and will be bound by the terms of purchase, and is jointly and severely liable with Client to Senior Care Solutions Online and its agents.
This agreement cannot be modified without the written consent of Senior Care Solutions Online. It may be terminated by any party, with or without cause, upon thirty (30) days written notice. These terms serve as an agreement and it is made in the State of Florida, it will be governed by the laws of Florida, and venue of any action to enforce it will be in Marion County, Florida. Should any legal action occur in were Senior Care Solutions Online and/or its agents are called to provide testimony or other information for legal action all expenses including travel and billable time are the responsibility of the responsible party.
4. SCSO PRIVACY POLICY
Pursuant to SCSO privacy policy, we may disclose to third parties certain aggregate information contained in your data, provided that, such information will NOT include personally identifying information, except as specifically authorized by you or in the good faith belief that such action is reasonably necessary to comply with the law or a legal process.
You understand that all information, data, text, software, photographs, graphics, messages or other materials ("Content"), whether publicly posted or privately transmitted, are the sole responsibility of the person from which such Content originated. This means that you, and not SCSO, are entirely responsible for all Content that you link to, email or otherwise transmit with. SCSO does not control the Content posted via the Service and, as such, does not guarantee the accuracy, integrity or quality of such Content. Under no circumstances will SCSO be liable in any way for any Content, including, but not limited to, for any errors or omissions in any Content, or for any loss or damage of any kind incurred as a result of the use of any Content posted, emailed or otherwise transmitted via the Service.
5. INTERNATIONAL USE
Recognizing the global nature of the Internet, you agree to comply with all local rules regarding acceptable Content. Specifically, you agree to comply with all applicable laws regarding the transmission of technical data exported from the United States or the country in which you reside.
6. INDEMNITY
You agree to indemnify and hold SCSO, and its subsidiaries, affiliates, officers, agents, co-branders or other partners, and employees, harmless from any claim or demand, including reasonable attorneys’ fees, made by any third party due to or arising out of your purchase, your use of Senior Care Solutions Online, your connection to SCSO, your violation of the terms, or your violation of any rights of another.
7. NO RESALE OF SERVICE
You agree not to reproduce, duplicate, copy, sell, resell or exploit for any commercial purposes, any portion of SCSO, use of the SCSO, or access to SCSO.
8. CHANGES/MODIFICATIONS TO SCSO
SCSO reserves the right at any time and from time to time to modify or discontinue, temporarily or permanently, SCSO service (or any part thereof) with or without notice. You agree that SCSO shall not be liable to you or to any third party for any modification, suspension or discontinuance of SCSO service.
9. LINKS
SCSO may provide, or third parties may provide, links to other World Wide Web sites or resources. Because SCSO has no control over such sites and resources, you acknowledge and agree that SCSO is not responsible for the availability of such external sites or resources, and does not endorse and is not responsible or liable for any Content, advertising, products, or other materials on or available from such sites or resources. You further acknowledge and agree that SCSO shall not be responsible or liable, directly or indirectly, for any damage or loss caused or alleged to be caused by or in connection with use of or reliance on any such Content, goods or services available on or through any such site or resource.
10. SCSO PROPRIETARY RIGHTS
You acknowledge and agree that SCSO and any necessary software used in connection with the Service ("Software") contain proprietary and confidential information that is protected by applicable intellectual property and other laws. You further acknowledge and agree that Content contained in sponsor advertisements or information presented to you through SCSO or advertisers is protected by copyrights, trademarks, service marks, patents or other proprietary rights and laws. Except as expressly authorized by SCSO or advertisers, you agree not to modify, rent, lease, loan, sell, distribute or create derivative works based on the Service or the Software, in whole or in part.
11. DISCLAIMER OF WARRANTIES
YOU EXPRESSLY UNDERSTAND AND AGREE THAT:
YOUR USE OF THE SERVICE IS AT YOUR SOLE RISK. THE SERVICE IS PROVIDED ON AN "AS IS" AND "AS AVAILABLE" BASIS. SCSO EXPRESSLY DISCLAIMS ALL WARRANTIES OF ANY KIND, WHETHER EXPRESS OR IMPLIED, INCLUDING, BUT NOT LIMITED TO THE IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE AND NON-INFRINGEMENT.
SCSO MAKES NO WARRANTY THAT (i) THE SERVICE WILL MEET YOUR REQUIREMENTS, (ii) THE SERVICE WILL BE UNINTERRUPTED, TIMELY, SECURE, OR ERROR-FREE, (iii) THE RESULTS THAT MAY BE OBTAINED FROM THE USE OF THE SERVICE WILL BE ACCURATE OR RELIABLE, (iv) THE QUALITY OF ANY PRODUCTS, SERVICES, INFORMATION, OR OTHER MATERIAL PURCHASED OR OBTAINED BY YOU THROUGH THE SERVICE WILL MEET YOUR EXPECTATIONS, AND (V) ANY ERRORS IN THE SERVICE WILL BE CORRECTED.
ANY MATERIAL DOWNLOADED OR OTHERWISE OBTAINED THROUGH THE USE OF THE SERVICE IS DONE AT YOUR OWN DISCRETION AND RISK AND THAT YOU WILL BE SOLELY RESPONSIBLE FOR ANY DAMAGE TO YOUR COMPUTER SYSTEM OR LOSS OF DATA THAT RESULTS FROM THE DOWNLOAD OF ANY SUCH MATERIAL.
NO ADVICE OR INFORMATION, WHETHER ORAL OR WRITTEN, OBTAINED BY YOU FROM SCSO OR THROUGH OR FROM THE SERVICE SHALL CREATE ANY WARRANTY NOT EXPRESSLY STATED.
12. LIMITATION OF LIABILITY
YOU EXPRESSLY UNDERSTAND AND AGREE THAT SCSO SHALL NOT BE LIABLE FOR ANY DIRECT, INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL OR EXEMPLARY DAMAGES, INCLUDING BUT NOT LIMITED TO, DAMAGES FOR LOSS OF PROFITS, GOODWILL, USE, DATA OR OTHER INTANGIBLE LOSSES (EVEN IF SCSO HAS BEEN ADVISED OF THE POSSIBILITY OF SUCH DAMAGES), RESULTING FROM: (i) THE USE OR THE INABILITY TO USE SCSO; (ii) THE COST OF PROCUREMENT OF SUBSTITUTE GOODS AND SERVICES RESULTING FROM ANY GOODS, DATA, INFORMATION OR SERVICES PURCHASED OR OBTAINED OR MESSAGES RECEIVED OR TRANSACTIONS ENTERED INTO THROUGH OR FROM SCSO; (iii) UNAUTHORIZED ACCESS TO OR ALTERATION OF YOUR TRANSMISSIONS OR DATA; (iv) STATEMENTS OR CONDUCT OF ANY THIRD PARTY ON SCSO; OR (v) ANY OTHER MATTER RELATING TO SCSO.
13. NOTICE
Notices to you may be made via either email or regular mail. SCSO may also provide notices of changes to the terms or other matters by displaying notices or links to notices to you generally on SCSO Homepage.
14. MARK INFORMATION
Senior Care Solutions Online, SCSO logo, products and service names are marks of Senior Care Solutions Online. Without SCSO prior permission, you agree not to display or use in any manner, ANY SCSO markings.
15. COPYRIGHTS and COPYRIGHT AGENTS
SCSO respects the intellectual property of others, and we ask our users to do the same.

16. GENERAL INFORMATION
The terms constitute the entire terms between you and SCSO and govern your use of SCSO. Terms are for informational purposes only and do not replace, and are not intended to replace, a written, mutually signed contract.. You also may be subject to additional terms and conditions that may apply when you use affiliate services, third-party content or third-party software. The terms and the relationship between you and SCSO shall be governed by the laws of the State of Florida without regard to its conflict of law provisions. You and SCSO agree to submit to the personal and exclusive jurisdiction of the courts located within the county of Marion, State of Florida. The failure of SCSO to exercise or enforce any right or provision of the terms shall not constitute a waiver of such right or provision. If any provision of the terms is found by a court of competent jurisdiction to be invalid, the parties nevertheless agree that the court should endeavor to give effect to the parties’ intentions as reflected in the provision, and the other provisions of the terms remain in full force and effect.
